Background. Hypertension, due to its high incidence rate, has become a major challenge to global public health. The stability of exosome miRNAs in body fluids and their ability to regulate the pathophysiology of hypertension indicate that exosome miRNAs are promising biomarkers for the detection and monitoring of hypertension. However, the role of exosome miRNAs in patients with hypertension complicated with left ventricular hypertrophy remains unclear.Objective. This study investigated the differential expression of miRNAs in plasma exosomes of patients with hypertension (HT) complicated with left ventricular hypertrophy (LVH) and their relationship with echocardiographic parameters, aiming to explore potential diagnostic biomarkers.Results. The miRNA sequencing results showed that a total of 3,512 miRNAs were generated, among which 2,275 were new miRNAs. Among them, the plasma-derived exosomes hsa-miR-320a-3p and hsa-miR-1908-5p were significantly upregulated between the control group and the experimental group. Finally, the analysis revealed that hsa-miR-1908-5p was linearly positively correlated with LVMI (B=0.046, P=0.014), linearly negatively correlated with LVIDd (B=-0.131, P=0.034) and IVSd (B=-0.349, P=0.013), and negatively correlated with LVEF (B=0.032). P=0.156) and LVPWT (B=0.114, P=0.553) were not correlated. The AUC value of hsa-miR-1908-5p combined with LVMI (0.948, P<0.001) was higher than the AUC values of the two alone, with a sensitivity of 90.5% and a specificity of 90%.Conclusion. The exosome hsa-miR-1908-5p derived from plasma is a valuable diagnostic biomarker for the early detection of hypertension combined with left ventricular hypertrophy, and it has a better predictive ability when combined with the LVMI.
