Questionnaires for Criteria Weight Derivation, Subsystem Rule Extraction, and Company Evaluation

The questionnaires were used in three main stages of the research. The first questionnaire was designed to derive the weights and rankings of the main IT governance criteria using the SWARA method. The second questionnaire was employed to extract knowledge-base rules for designing subsystem and integrated fuzzy expert systems. The third questionnaire was used to assess the IT governance status of the case study organization based on evaluations provided by IT managers.
