NRS-19030 | Records relating to Review of Child Deaths [NSW Ombudsman]

The series comprises of records relating to child deaths which have been reviewed by the NSW Ombudsman.The reviews concern the deaths of children at risk of harm, children in care or other persons in care as defined in legislation, currently Section 36 of the Community Services (Complaints, Reviews and Monitoring) Act 1993 ( Act No. 2, 1993).The Community Services Division joined the NSW Ombudsman office in December 2002.The files contain folders of paper reports and correspondence, and occasionally CD-ROMs and DVDs.The folders are prefaced with 'DDR' and a number and arranged in file number order. Prior to 2006, the files were entered into the Community Services Division Disability Death Review Database. From 2006 the files were entered into the Child Deaths Review database (CDR).
