A case of M. leprae rifampicin resistance (rpoB, S456L) after irregular anti-leprosy treatments since 1971 is reported. Whole-genome sequencing from four samples revealed a low genetic variation among the isolates suggesting relapses due to resistance and not reinfection, while a second compensatory mutation was detected (rpoC, L527V). Patient adheres to alternative therapy that allowed clinical improvement.
