Introduction: Pancreatic cancer is associated with a dismal prognosis, characterized by a 5-year survival rate of approximately 13%. However, cases of long-term survival have been observed with multidisciplinary treatment. Although bone metastasis may occur, metastasis to the mandible is extremely rare. This report presents a case of pancreatic cancer with mandibular bone metastasis. Case Report: An 88-year-old man with a history of pancreatic cancer presented to a dental clinic with swelling on the left side of his cheek and was referred to our department for further examination. Physical examination revealed significant swelling of the left cheek and a bulge in the mandibular bone. The patient experienced paresthesia of the left lower lip but had no trismus or oral ulceration. Computed tomography showed bone destruction at the left mandibular angle. Blood tests revealed a CA19-9 level exceeding 12,000 U/mL. Because the patient was already receiving best supportive care, palliative treatment was initiated, including external radiation therapy (20 Gy over 5 sessions) and opioid administration. The patient’s condition deteriorated, leading to death. Conclusion: This case is extremely rare, and in cases with a short survival expectancy, minimally invasive palliative treatment is preferable.
