A 77-year-old woman developed hyperkeratotic nodules and plaques on both legs and feet. Initial histopathological findings suggested well-differentiated squamous cell carcinoma (SCC). However, clinical-pathological reevaluation confirmed a diagnosis of hyperkeratotic lichen planus (HLP) with epitheliomatous hyperplasia (PEH). Treatment with topical corticosteroids resulted in significant improvement. This case underscores the importance of careful differentiation between HLP and SCC to avoid unnecessary surgical intervention.
