Information recorded includes date of consent for fostering, child’s name, child’s date of birth, birth mother’s name, foster mother’s name, mother’s admission date, child’s discharge date, social worker’s initials, and remarks which generally outline the result of the fostering (either adopted or returned to birth mother).
