Natural environments, anthropogenic pollution, nutrition, and their interactions are likely leading causes of human diseases. The determinants of these diseases exhibit distinct spatial distributions across geographic units, thus thorough studies of these determinants require analyses of relevant geographic layers. Based on spatial variability analysis of geographic layers, we propose four geographic detectors for assessing environmental health risks: the Risk Detector identifies the locations of high-risk areas; the Factor Detector identifies the factors contributing to risks; the Ecological Detector reveals the relative importance of different factors; and the Interaction Detector determines whether risk factors interact synergistically or act independently to induce diseases. In a real-world case study, major physical environments (including watersheds, rock zones and soils) were found to strongly control the incidence of neural tube defects (NTDs) in the Shun'an region, China. Basic nutrition (derived from food) plays a more significant role than anthropogenic pollution (such as fertilizers) in shaping the spatial patterns of NTDs. Ancient substances released from geological faults subsequently spread along slopes, greatly elevating the risk of NTD dissemination. These findings offer valuable insights for developing disease intervention strategies in relevant regions.
