It provides key surveillance indicator estimates for chronic diseases and their risk factors at the national and state levels across the United States. The data can be disaggregated by gender, race, ethnicity and age group, to support evidence-based decision-making and the formulation of activities, programs and policies aimed at improving population health. The data is presented in the form of maps, bar charts, line graphs and tables, and is also directly accessible via the CDI Data Portal.
