This series was used to record reports on injuries sustained by patients. The reports were written by the medical superintendent. They are usually about one paragraph in length.Details recorded include:name of patienthow the accident occurredan account of injuries diagnoseda call for explanations. Explanations were not recorded in this volume, in most cases there is only a small note to say the explanation was accepted.The entries are also signed by the Inspector-General for the Insane.Some memoranda documenting explanations from staff for one particular case were found within the volume.
