This final version of the questionnaire inquired patients, in a 1-5 anchored Likert scale (1- very poor, 2- poor, 3- reasonable, 4- good, 5- very good), about the comfort of the waiting room (8 items: illumination, noise, chair’s comfort, feeling of privacy, ability to talk with caregivers or other patients, unrestrained use of mobile phone, distractions/amenities -TV, magazines or others-, and patients’ perception of waiting time) and the consultation room (7 items: illumination, good visualization of the doctor’s face, chair’s comfort, understanding of the topics discussed with the doctor, computer’s presence during the consultation, feeling of privacy during the conversation, and feeling of privacy during the physical exam). It also had an open question for collecting suggestions and comments. This questionnaire was used for assessing the old design (step 1) and the new design (step 2) of the consultation room; in addition, participants of step 2 were also invited to compare, in a 1-5 Anchored Likert scale (1-substantially worst, 2- worst, 3- the same, 4- better, 5- substantially better) the comfort of the traditional consultation room with that of the re-designed setting. Patients’ filled the questionnaire in the waiting room, unaided, immediately after their consultation.
