The Hospital Outbreak Resilience Index (HORI) Montgomery County 6-Axis Resilience Inventory is a facility-level dataset of 60 licensed healthcare facilities in Montgomery County, Maryland, scored across six axes of resilience exposure under combined infrastructure stress and outbreak scenarios. The dataset connects healthcare facility quality data to electric grid infrastructure age and reliability data, producing an integrated view of facility risk that no existing public dataset previously made available at this level of granularity. The 60 facilities span seven institutional types: 35 Skilled Nursing Facilities, 13 Federally Qualified Health Centers, 6 Acute Care Hospitals, 2 Rehabilitation Hospitals, 2 Hospices, 1 Federal Military Hospital (Walter Reed National Military Medical Center), and 1 Federal Research Hospital (NIH Clinical Center). Each row represents one facility scored on six axes: Clinical Surge Capacity, Infection Control Infrastructure, Power Resilience, Grid Stress Exposure, Community Outbreak Risk, and NPSG Compliance Baseline. The v1.1 release adds substantial Axis 2 (Infection Control) population to 45 of 60 facilities through integration of CDC National Healthcare Safety Network (NHSN) Healthcare-Associated Infection Standardized Infection Ratios for hospitals (MRSA, C. difficile, CLABSI, CAUTI), CMS Nursing Home Compare infection control deficiency citations for skilled nursing facilities, the Leapfrog Group Hospital Safety Grade including the Spring 2026 release that first extended grades to federal facilities, and Maryland hospitalization rate per 1,000 resident days. Key findings: Eight facilities are served by the Pepco Chevy Chase substation, built in 1952 and currently 34 years past the FERC 40-year recommended replacement cycle. These eight include Suburban Hospital (highest Medicare discharge volume in MoCo), Walter Reed, NIH Clinical Center, and five skilled nursing facilities. All 35 Skilled Nursing Facilities in the dataset are served by substations rated HIGH or CRITICAL. None are served by substations within the FERC replacement cycle. The Jewish Social Service Agency Hospice has the highest combined vulnerability score in the dataset: 892 Medicare beneficiaries at average age 88, 24-hour estimated generator fuel storage, and a community vulnerability score of 9.0. All 13 Montgomery County Federally Qualified Health Centers serve overburdened communities, qualify for IRA Section 6417 Direct Pay as tax-exempt entities, and average the lowest power resilience scores of any facility class in the dataset. The dataset is intended for use by researchers, policy analysts, emergency management planners, healthcare quality teams, and infrastructure investors examining the intersection of healthcare resilience, grid stress, and community vulnerability. Specific applications include Maryland Energy Administration Resilient Maryland program planning, county-level resilience hub designation, foundation grant prioritization for healthcare facility upgrades, and academic research on the combined infrastructure-and-outbreak scenario that critical care clinicians and emergency management directors increasingly face. The accompanying methodology note (Zenodo DOI 10.5281/zenodo.20313369) documents the data source selection rationale, the facility-type-specific scoring approach, and the empirical impact of CDC NHSN HAI data integration on facility ranking. Researchers using this dataset for academic citation should cite both the dataset and the methodology note. Known limitations: Axis 1 (Clinical Surge Capacity) remains null for all 60 facilities pending primary data collection from facility clinical leadership. Axis 2 remains null for FQHCs, hospices, and the NIH Clinical Center due to lack of applicable public infection control data. Generator capacity and fuel hours are class-norm estimates rather than facility-verified measurements. Substation assignments are based on geographic proximity rather than confirmed Pepco circuit maps (a pending Maryland PSC MPIA request will produce confirmed assignments). The dataset is point-in-time with source data extracted April and May 2026. This dataset is released under Creative Commons Attribution 4.0 International (CC BY 4.0). Attribution is required for any use or redistribution.
