We present a rare APL case with negative result of karyotype and commercially RT-PCR detecting PML-RARA. However, FISH analysis revealed an insertion signal of RARA into PML on one cytogenetically normal chromosome 15. The discrepancy between commercial RT-PCR and FISH led to an additional study by mRNA-seq identifying one new PML-RARA isoform.
