These forms were originally enclosed in a folder labelled "Samples of Printed Forms in Current Use for Office Reference Only. Not to be Used". Two copies of each form were kept, one of which might be sent as a sample when ordering if none were in stock. The forms cover all aspects of the administration of the hospital - medical, staff, accounting and stores. Some forms appear in several different versions since when the form was changed the superseded version was not removed from the folder. Also included is a Stores Supply Department circular on the wholesale rates of benzine and petrol dated 18 July 1922. (7/5347). 1 box.
