A purpose-built questionnaire framed in part by components of the Canadian Institutes of Health Research (CIHR) definition of knowledge translation including multiple choice, 5-6 item Likert scale response and/or open-ended questions in the following categories: 1) demographic and work characteristics; 2) practices related to knowledge translation and research impact communication; and 3) knowledge, confidence and attitudes about the importance of KT and communicating research impact.
