These are 13 estray loose leaf forms for Infectious Diseases Notification and Prevention under the Public Health Act of 1921, patients Schedule D (60 Vic. No. 38, Part III). This is not a complete series, the forms cover part of the period 17 November 1931 to 7 August 1933.These notifications were sent to the Head Teacher of the School, and record the child’s name, sex and age, which Local Authority has been notified, where the child is ill, and what infectious disease.Infectious diseases including diphtheria, scarlet fever, meningococcal meningitis, typhoid, infantile paralysis, bubonic plague and smallpox are listed as well as their period of exclusion. Children were excluded from school for 2 months if suffering from an infectious diseases, or 6 weeks if residing in a house where a disease existed. A certificate from a medical practitioner was required to state the child was free from disease and infection and that their clothes were satisfactorily disinfected.
