<b>Background:</b> Cataract surgery is the most common surgery on the eye and to get the desired result needs to control inflammation and reduce postoperative inflammation and its consequences, including corneal edema and cystoid macular edema, therefore topical corticosteroid drops should be used for the first few weeks after the operation to control inflammation, but these drugs might change the intraocular pressure.<b>Objectives:</b> This study compares the effect of topical loteprednol etabonate 0.5% and dexamethasone 0.1% on intraocular pressure (IOP) in post-cataract surgery.<b>Patients and Methods:</b> This prospective study involved patients who underwent cataract surgery and were randomly divided into two groups; Group A received loteprednol etabonate 0.5% drops postoperatively, while Group B received dexamethasone 0.1% suspension postoperatively. The intraocular pressure was measured preoperatively and was rechecked one week and four weeks after the surgery.<b>Results:</b> Sixty-six eyes of 66 patients were enrolled (33 patients in group A and 33 in group B) with no clinically significant differences related to age or gender between the two groups, the intraocular pressure was 16.45 ± 3.00, and 16.42± 2.76 mmHg in group A and B respectively preoperatively which was clinically insignificant with p-value 0.522 and decreased slightly after the surgery 15.82± 3.005 and 15.61± 2.817mmHg in group A and B respectively after one week postoperatively but also clinically insignificant and was 15.58± 2.211 and 16.55± 4.452 mmHg in group A and B after four weeks postoperatively and all these changes in IOP was clinically insignificant.<b>Conclusion:</b> There is no difference between topical loteprednol etabonate 0.5% and dexamethasone 0.1% on intraocular pressure (IOP) in post-cataract surgery.
