Hypervirulent Klebsiella pneumoniae (hvKp) is a new variety of K. pneumoniae that can cause serious infections to immunocompetent hosts. HvKp causes primary infections such as meningitis, necrotizing fasciitis, endophthalmitis and osteomyelitis, which are more likely to spread. In previous researches, hvKp was usually sensitive to most antibiotics including cephalosporins and carbapenems. However, recently there has been the emergence of multidrug-resistant hvKp (MDR-HV). For hvKp, acquisition of polymyxin resistance can be mediated by the mcr-1 plasmid or the PhoP-PhoQ-Arn pathway. In another way, classical K. pneumonia (cKp) strains can enhance virulence through virulence plasmids to become MDR-HV. The combination of high virulence and multidrug resistance of K. pneumonia has attracted more and more attention.In order to explore the linked mechanism between high virulence and multidrug resistance of K. pneumoniae, we collected polymyxin B-resistant K. pneumoniae isolates from Xiangya hospital and conducted in-depth investigations of their clinical, molecular and virulent characteristics in this study.
