Anaplastic lymphoma kinase (ALK) inhibitors will soon be used alongside chemotherapy in upfront treatment of ALK aberrant high-risk neuroblastoma. However, resistance is being observed particularly after ALK monotherapy. Biomarkers of response need to be determined alongside novel combinations to overcome acquired resistance. In this study we used various pre-clinical models of ALK aberrant neuroblastoma to test ALK inhibitors focusing on lor-latinib, alone and in combination with either conventional chemotherapy or the small molecule MDM2 inhibitor idasanutlin. We found synergy in pre-clinical in vivo models with high basal ALK expression and recommend ALK expression is measured in patients receiving ALK inhibitor combinations for ALK aberrant neuroblastoma in future clinical trials.
