A wearable skin hydration sensor was developed using silver nanowire (AgNW)/polyborosiloxane (PBS) composites, combining self-adhesion and self-healing in a single material system. PBS's reversible Si-O-B dynamic covalent bonds enable intrinsic mechanical and electrical recovery — restoring full mechanical properties within three hours and 77% electrical conductivity within 24 hours after damage. The substrate's low Young's modulus (~240 kPa) and high stretchability (~600%) ensure conformal skin contact without additional adhesives, while outperforming commercial ECG patches in wet-condition adhesion. Electrochemical impedance spectroscopy validated accurate hydration sensing, including real-time tracking during exercise. This work establishes key design principles for robust, conformable wearable sensors for continuous health monitoring.
