**Overview** This data package contains information about a list of Behavioral Risk Factor Health and Diseases Surveillance System over health-related quality of life, healthy aging data and the information regarding the Selected Metropolitan Area Risk Trends (SMART) project and Behavioral Risk Factor Surveillance System (BRFSS) responses as to the access healthcare from 1995-2011. **Description** This data package contains the Behavioral Risk Factor Surveillance System (BRFSS) is a continuous, state-based surveillance system that collects information about modifiable risk factors for chronic diseases and other leading causes of death. Health-Related Quality of Life (HRQOL) surveillance is used to identify unmet population health needs including recognizing trends, disparities, and determinants of health in the population. Self-assessed health status is also a more powerful predictor of mortality and morbidity than many objective measures of health. HRQOL measures make it possible to demonstrate scientifically the impact of health on quality of life, going well beyond the old paradigm that was limited to what can be seen under a microscope. The healthy aging data by Centers for Disease Control and Prevention (CDC) comprises of data on a range of key indicators of health and well-being, screenings and vaccinations, and mental health among older adults at the national and state levels. These indicators provide a snapshot of currently available surveillance information, and can be useful for prioritization and evaluation of public health interventions. Selected Micropolitan Area Risk Trends (SMART) uses BRFSS data to provide prevalence rates for selected conditions and behaviors for cities and their surrounding counties. The risk factor categories available are alcohol consumption, asthma, cholesterol awareness, colorectal cancer screening, diabetes, disability, exercise and physical activity, health status, health care access, hypertension awareness, immunization, nutrition, oral health, tobacco use, weight classifications, and women’s health. The prevalence and trends health care access documentation provides technical and statistical information regarding the BRFSS responses as to the access and coverage to health care of people in the U.S. The BRFSS is the nation's premier system of health-related telephone surveys that collect state data about U.S. residents regarding their health-related risk behaviors, chronic health conditions, and use of preventive services. This prevalence data on Tobacco Use among adult U.S. residents are used to design programs that promote preventive health practices that can affect their health status. The purpose of the 500 Cities Project is to provide city and census tract-level small area estimates for chronic disease risk factors, health outcomes, and clinical preventive service use for the largest 500 cities in the United States. These small area estimates will allow cities and local health departments to better understand the burden and geographic distribution of health-related variables in their jurisdictions, and assist them in planning public health interventions. The Vision and Eye Health Surveillance System is intended to provide population estimates of vision loss function, eye diseases, health disparities, as well as barriers and facilitators to access to vision and eye care.
