This qualitative study investigated the experiences of caregivers of patients with Alzheimer's disease and related dementias (ADRD) in Uganda's Wakiso District, specifically in the Nansana and Busukuma sub-counties. Fourteen caregivers, representing diverse age and sex groups, were selected through purposive sampling and participated in in-depth interviews conducted in Luganda. A thematic analysis of the interviews revealed significant insights into caregiver selection, struggles, and benefits. Caregiver Selection and Struggles Caregiver selection is deeply influenced by pre-existing familial relationships, with emotional connections and patient preferences playing pivotal roles. Caregivers are often chosen based on parent-child dynamics, sibling bonds, and marital ties, reflecting a sense of duty and emotional ties to the patient. Caregivers face substantial emotional, physical and financial challenges. Emotionally, they experience role reversal of caring for once-strong family members, leading to feelings of helplessness and isolation. Physically, they manage demanding daily care tasks, including helping with mobility and hygiene, which often result in personal health issues and loss of appetite. In particular, caregivers bear significant costs for medical care and other needs, depleting their savings and sacrificing employment and educational opportunities. These multidimensional struggles highlight the critical need for enhanced support systems and resources for caregivers in low-resource settings. Benefits and Implications for Caregiving Support Despite these challenges, caregivers experience various benefits. They acquire valuable skills and knowledge, particularly in navigating medical networks and obtaining professional guidance, which leads to better patient care. Caregiving fosters a profound sense of empathy and understanding, with caregivers developing greater tolerance, patience, and the ability to offer unconditional love and support. These emotional enrichments contribute to personal growth and a deeper appreciation of life and human relationships. Caregivers also experience a sense of reciprocity and gratitude and feel fulfilled by giving back to those who once supported them. Community and external support are crucial in buffering caregiving stress and providing financial, emotional, and spiritual assistance that creates a more supportive environment. This study underscores the need for targeted interventions to support caregivers of ADRD patients, especially in resource-limited settings such as Uganda. Training programs, support groups, and healthcare policies must consider cultural and societal nuances that influence caregiving. Addressing both the challenges and benefits identified can improve care outcomes for patients with ADRD and enhance the well-being and support systems for their caregivers
