This series consists of records, interviews, and questionnaires from the State Rail Authority Inquiry into Management Accountability and Controls. It also includes documents resulting from the inquiry: Review, Report of the inquiry. Consignment 2 contains audio cassettes of reports and interviews, and computer disks containing data from the inquiry.The Wallis Inquiry was set up in early 1993 as a result of the ICAC [Independent Commission Against Corruption] into the Northern Region of Freight Rail. ICAC required State Rail to provide a report as to who, if anyone, should be held responsible for the breakdown in the project management of earthworks undertaken in that region from 1988 onwards. Stewart Wallis AO, former Chief Executive of Leightons, was appointed by the State Rail Board in April 1993 to conduct the inquiry with the full support of the Board, the Chief Executive and Management. The Inquiry commenced in May 1993. A detailed questionnaire was sent to more than 40 senior State Rail managers, and these were followed up with interviews of 20 managers, some who had left State Rail at the time. The Inquiry also collected and received a large amount of material from both management and the Task Force investigating the Northern Region fraud. ICAC originally requested a report by June 30 1993, but several extensions were allowed. A final report was presented at the September 1993 Board meeting, and the Chief Executive forwarded his report to ICAC shortly afterwards.Custody History From 1955 until 2000 the State Rail Authority of New South Wales maintained an 'in house archives' consisting of the archival records of the Authority, its predecessor agencies and the private railway companies taken over by the government. The date ranges of the records were from 1849 until 2000. When the decision was made to close the 'State Rail Archives' the parts of the collection that were required as State archives were transferred to the State Records Authority of NSW. Those record series were incorporated into the State Records series system of archives control and were re-numbered. This series was formerly part of the State Rail Archives collection where it was identified as series R525. It may have been viewed in the State Rail Archives as series R525 and could have been previously cited as such in notes by researchers at State Rail Archives or in publications or unpublished works.
