We conducted a 24-week, prospective, single-arm, pre-post intervention pilot study at a community oncology practice (Pacific Cancer Care, Monterey, CA, USA) to evaluate the effects of a plant-based dietary intervention. Each participant served as his own control to evaluate changes in PSA kinetics, cardiometabolic markers, dietary intake, IGF-1 and IGFBP-3. Eleven men were enrolled, and ten completed the study divided into five cohorts of one to three men each. One participant was discontinued midway through study due to disease progression. Study activities occurred between March 2024 and September 2025.Participants attended twelve 1.5-hour group sessions over the 24-week study period. Sessions were held weekly during the first nine weeks, and then at weeks 12, 18, and 24. Each session included nutrition education emphasizing plant-based dietary patterns, along with plant-based cooking demonstrations and taste testing. The mobile teaching kitchen utilized a hot plate and toaster oven; classes took place in the waiting room after hours and in the conference room during business hours.Participants were encouraged to obtain the majority of their caloric intake from fruits, vegetables, whole grains, legumes, nuts, and seeds, while limiting or avoiding animal products and processed foods, including meat, poultry, fish, dairy, and eggs. To support dietary adherence, participants received weekly organic produce boxes during weeks 1 through 9 and $20 grocery store gift cards from weeks 10 through 24. Each participant also received an educational booklet containing nutritional information and recipes.Strict adherence to the plant-based diet was not required to allow dietary flexibility. All participants were instructed to take a vitamin B12 supplement throughout the study. A support person (e.g., spouse) was encouraged, but not required, to attend sessions. Meetings were led by a certified health coach and nutritionist (D.A.T.).
