<h3>Background</h3>
This collection contains documentation from key events throughout the project, including kick-off meetings, working conferences, reflection and peer review sessions for teams T1, T2, and T3. Spanning from October 2021 to March 2023, the materials—organized in over 90 PDF files—capture the collaborative design, development, and evaluation processes of hybrid care pathways. Each folder reflects a distinct phase of co-creation, learning, and progress within the Zorg Vooruit project.

<h3>Purpose</h3>
To support the development of blended care professionals (CPs) by documenting and reflecting on the co-creation, implementation, and evaluation of hybrid rehabilitation pathways.

<h3>Method</h3>
Action research, involving iterative cycles of co-creation, implementation, reflection, and peer review across multidisciplinary teams (T1–T3).

<h3>Contextual Note</h3>
Rijndam’s Zorg Vooruit+ program, supported by ZonMw, focuses on developing hybrid physical-and-digital rehabilitation pathways for patients recovering from stroke or spinal cord injury. Key highlights include:
- Blended care: combining in-clinic services with online support.
- Collaborative networks: involving rehabilitation centers, first‑line practitioners, and Erasmus University researchers.
- Co-creation & reflection: jointly building and learning from these hybrid care designs.

This project aligns with ZonMw’s increasing emphasis on digital integration and patient-centered, value-driven healthcare.
