This study aimed to explore the longitudinal trajectories of postoperative kinesiophobia and their impact on activities of daily living (ADL) in elderly orthopedic patients. A prospective longitudinal design was adopted, with kinesiophobia (measured by TSK) and ADL (measured by Barthel Index) assessed at multiple time points postoperatively. Latent class growth modeling identified distinct kinesiophobia trajectory classes (e.g., persistent high, resolving, moderate stable), and further analysis revealed that patients with persistent high kinesiophobia had significantly poorer ADL recovery during follow-up. These data clarify the dynamic changes of postoperative kinesiophobia and its clinical impact, providing a basis for targeted intervention to improve functional outcomes in elderly orthopedic patients.
