Readiness in dementia

This project is about readiness for advance care planning of people with dementia, their family caregivers, and their health care professionals (HCP), and the use of conversation aids. The HCPs participated in an intervention study where they first followed an elaborate elearning on readiness and conversation aids, and following, they had ACP conversations with their patients and family. The data consists of pre-intervention questionnaires about readiness and use of conversation aids, and the post-test after the ACP conversations was on the same topic. In this ZENODO record you will find the study protocol, the data management plan, the informed consent forms for patients, family and HCPs, and the datadictionairies for six different questionnaires (per-post test).
