We developed a novel quantitative method for assessing varus thrust during walking using acceleration data acquired from inertial measurement units (IMUs). This study aimed to examine the reliability of the developed metrics and evaluate their ability to discriminate between patients with knee osteoarthritis (OA) exhibiting varus thrust and healthy adults. Overall, 16 knee OA patients and 16 healthy adults walked on a treadmill, with IMUs attached to the tibial tuberosity and the lateral femoral condyle. As an index of varus thrust, we calculated the root mean square (RMS) acceleration in the mediolateral direction. This value was adjusted by dividing it by the walking swing speed to obtain the adjusted RMS (A-RMS), as the RMS of acceleration is strongly coupled with movement velocity. The intraclass correlation coefficients (ICC) of A-RMS for the tibia and femur were 0.85 and 0.73, respectively. Significant differences in A-RMS were observed between knee OA patients and healthy adults for both the tibia and femur, with large effect sizes (Cohen's d: 1.23 and 0.97, respectively). Our results indicate that A-RMS exhibits excellent test-retest reliability and can effectively distinguish between knee OA patients with varus thrust and healthy adults.
