Quality and Safety in Transitional Care of the Elderly, 2015

The aim of this project was to study how quality and safety of patients within EHCS is dependent on different types of transitions and interactions between service providers. The conceptualisation of the quality and safety of EHCS comprised three components: clinical effectiveness, patient safety and patient experience. The overall aim of the research study was to understand coordination aspects of significance for the quality and safety of EHCS in Norway. Data was collected through survey, observation and interviews.
