This series consists of patient admission sheets from Maitland Hospital dating from November 1933 to December 1957. The admission sheets are the only record of patient admissions to Maitland Hospital during this period.The admission sheets are printed forms [Form AI, admission information] which have been filled out, in most cases, by hand. Each admission sheet records the patient's details, including the patient's name, birth date and location. The date of admission and date of discharge of the patient is given, the doctor attending, diagnosis and result of treatment are noted on each admission sheet. During this period babies born at the hospital were also admitted using the hospital admission sheets.The admission sheets have been bound in volumes that cover, in most cases, a period of three or four years. However the later volumes contain records covering a period of only two years.Within each volume the sheets have been arranged alphabetically. The majority of the volumes consist of only male or only female admission sheets for the dates covered. However, in some cases both male and female records are bound in the same volume.Additional information recorded on each admission sheet (where available) includes, the patient's marriage date and location, the name of the patient's spouse, the names and ages of the patient's children, the patient's occupation, employer, wages and information regarding health funds and pensions. Fees charged and payment details are also given in some cases.
