Suicidal Concern in Adolescents with Attenuated Psychosis Syndrome: A Categorical and Dimensional Approach

Introduction This database includes the raw data linked with the paper “Suicidal Concern in Adolescents with Attenuated Psychosis Syndrome: A Categorical and Dimensional Approach“. In this paper, we report on the clinical, sociodemographic data and result from Columbia-Suicide Severity Rating Scale (C-SSRS), Comprehensive Assessment of At-Risk Mental States (CAARMS), Social and Occupational Functioning Assessment Scale (SOFAS), and Clinical Global Impression-Severity (CGI-S). The primary aim of the present study was to examine whether APS status is associated with the presence of suicidal concern (SC), defined as the presence of any suicidal ideation and/or behaviors assessed during the C-SSRS, in a clinical cohort of hospitalized adolescents, compared with adolescent inpatients not meeting APS criteria or psychosis. Secondary aims were to determine whether the dimensional severity of attenuated positive psychotic symptoms, indexed by the Comprehensive Assessment of At-Risk Mental States (CAARMS) total score, predicts SC independently of categorical APS status and to evaluate the contribution of sex at birth and age to suicide risk. As a sensitivity analysis, we further examined the association between APS status, CAARMS total score, and ordinal suicide risk severity (NSC < Low < High). Methods This cross-sectional study included 100 adolescents admitted to a tertiary child neuropsychiatry inpatient unit between 2020 and 2024. APS was assessed using the Comprehensive Assessment of At-Risk Mental States (CAARMS), while suicidal ideation and behaviors were evaluated through the Columbia-Suicide Severity Rating Scale (C-SSRS). Multivariable logistic and ordinal regression models examined the association between APS, dimensional severity of attenuated psychotic symptoms, and suicidality. Results Forty-eight participants met APS criteria, and 67% reported SC. APS status was independently associated with increased odds of SC (OR=3.90, 95% CI 1.29–12.90, p=0.019), controlling for age, sex, and CAARMS total score. Greater dimensional severity of attenuated psychotic symptoms also predicted SC (OR=1.17, 95% CI 1.04–1.33, p=0.015). Female sex was associated with higher suicide risk, whereas age was not significant. Sensitivity analyses confirmed that CAARMS total score predicted increasing suicide risk severity.
