Dataset for: Phenotyping of older adults with chronic illness using clinical and pharmacotherapeutic data in a resource-limited tertiary care setting

STUDY OVERVIEW This dataset contains the preprocessed categorical clinical, socio-demographic, and pharmacotherapeutic features used for k-modes clustering and phenotyping of older adults with chronic illnesses. The data was collected between January 2025 and February 2025 in the outpatient department of a 500-bedded tertiary care government hospital in central Nepal. STUDY DETAILS Sample Size: 360 participants (aged 65 years and above, diagnosed with at least one chronic illness, on treatment for at least 3 months) Methodology: Cross-sectional design using a non-probability purposive sampling framework. Continuous clinical and pharmacotherapeutic indicators were categorized using clinical domain thresholds or median splits to eliminate statistical noise and prepare the data for k-modes clustering metrics. Ethical Approval: Approved by Shree College of Technology Institutional Review Committee (SCoT-IRC) (Ref: SCoT-IRC-20250108-01). DATA DICTIONARY & FEATURE CODING The dataset consists of 19 columns: CODE: Unique patient identification number (1 to 360). Gender: Male / Female. Education: Illiterate / Primary / Higher. Marital_Status: Married / Widowed / Single. Living_With: Son/Daughter-in-law / Spouse / Alone / Others. Employment: Unemployed / Retired / Employed. Allowance: Government / None (Old-age social security benefit eligibility status). Monthly_Income: NRs. 10,000 and less / More than NRs. 10,000. Patient_Reported_Disease: Primary impactful chronic disease (HTN: Hypertension, DM: Diabetes Mellitus, COPD: Chronic Obstructive Pulmonary Disease, Others). Hospitalization: Self-reported annualized frequency (Less than once / Once or more). Age_Category: Young-Old (Less than 75 years) / Old-Old (75 years and above). Multimorbidity: Presence of 2 or more chronic conditions (Yes / No). Polypharmacy: Concurrent use of 5 or more medicines (Yes / No). Encounter_with_PIM: Exposure to at least one potentially inappropriate medication via 2023 AGS Beers Criteria (Yes / No). CCI_Category: Age-adjusted Charlson Comorbidity Index score severity (Mild / Moderate / Severe). Medication_Adherence: Evaluated via the Nepalese General Medication Adherence Scale (High / Good / Partial / Low / Poor). MRCI_Category: Medication Regimen Complexity Index split by median values (Low / High). Medication_use_Duration: Duration treating the primary illness split by median values (Short / Long). HRQoL: Health-Related Quality of Life via EQ-5D-3L and EQ-VAS median splits (Low / High).
