Low Birth Weight:
A child born weighing less than 5 pounds 8 ounces is considered to have a low birthweight by the Centers for Disease Control and Prevention. Children born below that threshold are at elevated risk for adverse health conditions and infant mortality. This indicator is the percentage rate of births with low birth weight out of all births, disaggregated by race and ethnicity.
Preventable Hospital Stays:
This indicator reports the rate of preventable hospital stays, defined as the number of discharges for ambulatory-care sensitive conditions per 100,000 Medicare enrollees.
Data is from the County Health Rankings & Roadmaps (CHR&R), a program of the University of Wisconsin Population Health Institute.
