Use this tool to learn if USAC has received your FCC Form 466 and collected the information it needs to make a funding commitment decision. If we are missing information, you will receive an Information Request once your form has been assigned to a reviewer. The tool's dataset refreshes daily. If your form status does not appear in the report more than 5 days after submitting your FCC Form 466 in MyPortal, please contact the RHC Help Desk via email or at (800) 453-1546.
