Enkopresis Encopresis

Enkopresis wird als funktionelles Einkoten ab dem Alter von 4 Jahren definiert und betrifft 1-3% aller Schulkinder. Die beiden wichtigsten Formen sind die Enkopresis mit und ohne Obstipation, im Kleinkindesalter zusätzlich das Toilettenverweigerungssyndrom. Komorbide psychische Störungen und Einnässen sind häufig. Aktuelle Befunde zur Ätiologie, Diagnostik und Therapie werden in dieser Übersicht dargestellt. Therapeutisch ist ein symptom-orientiertes, verhaltenstherapeutisches Vorgehen in Form eines Toilettentrainings am wirksamsten, ergänzt durch Laxantien (Polyethylenglykol) bei der Obstipation. Biofeedback ist nicht wirksam und andere Psychotherapien sind nur bei komorbiden psychischen Störungen indiziert. Der Langzeitverlauf war bisher ungünstig und muss verbessert werden. The two most important subtypes are encopresis with and without constipation. In preschoolers toilet refusal syndrome can occur. Comorbid behavioural disorders and urinary incontinence are common. The current state-of-the-art regarding aetiology, assessment and therapy is presented in this overview. A symptom-oriented behavioural approach (toilet training) is most successful, with the addition of laxatives (polyethylene glycol) if constipation is present. Biofeedback is not effective. Other forms of psychotherapy are indicated only in case of comorbid behavioural disorders. The long-term outcome has been poor and needs improvement.
