Introduction: Therapeutic alliance is a well-established predictor of psychotherapy outcomes; however, limited research has examined how congruence between therapist and client perceptions of the alliance influences psychological well-being, particularly within non-Western cultural contexts. This study investigated the clinical significance of therapist–client alliance congruence in predicting client psychological well-being among Filipino psychotherapy dyads.Methods: The study involved 104 Filipino client–therapist dyads engaged in psychotherapy. Therapist- and client-rated alliance were analyzed using polynomial regression with response surface analysis to model both congruent and discrepant alliance effects on client psychological well-being. Treatment length, treatment interval, therapist professional maturity, and client symptom severity were included as covariates.Results: Results indicated a significant positive linear effect along the line of alliance congruence, such that a mutually perceived alliance was associated with greater psychological well-being. In contrast, significant curvature along the line of incongruence showed that discrepancies between therapist and client alliance ratings were associated with lower well-being, with increasing mismatch predicting progressively poorer outcomes. Notably, maximum psychological well-being was observed under conditions of high mutual alliance, particularly when clients perceived the alliance more positively than their therapists did.Conclusion: The results underscore the relational and culturally embedded nature of the therapeutic alliance and suggest that clinicians and researchers should attend to perceptual alignment within the therapeutic dyad. Implications for alliance-informed practice and culturally responsive psychotherapy research are discussed.Keywords: alliance congruence, well-being, polynomial regression, response surface analysis
