We retrospectively analyzed 109 patients with Stage IV HER2-negative GC who received first-line PD-1 inhibitors combined with chemotherapy at our center between January 2020 and December 2025. Clinicopathological data, including PD-L1 expression (CPS) and baseline nutritional indicators (albumin, prealbumin, BMI, hemoglobin), were collected. Progression-free survival (PFS) and overall survival (OS) were assessed using Kaplan–Meier method and Cox regression models. A nomogram was constructed to predict survival probabilities.
