Acute myeloid leukemia (AML) patients in remission remain at risk of relapse even after allogeneic hematopoietic cell transplantation (alloHCT). AML measurable residual disease (MRD) status prior to alloHCT has been shown to be prognostic. It is unknown if modulating the intensity of the alloHCT conditioning regimen in AML patients testing positive for MRD can prevent relapse and improve survival. Ultra-deep error-corrected sequencing for 13 commonly mutated genes in AML was performed on pre-conditioning blood from patients treated on a phase III clinical trial randomizing adult myeloid malignancy patients in complete morphological remission to myeloablative (MAC) or reduced intensity conditioning (RIC). This goal of this study was to determine the impact of alloHCT conditioning intensity on AML patients in morphological CR but with genomic evidence of residual disease.
