Characterization of the Intrahepatic Microbiome in Patients with HBV-Related End-Stage Liver Disease

This study investigates the intrahepatic microbiome in hepatitis B virus (HBV) related end stage liver disease (ESLD). Liver tissue samples from 20 patients with decompensated cirrhosis (DC), 24 with acute on chronic liver failure (ACLF), and 18 healthy donors (total n=62) were analyzed by metatranscriptomic sequencing (~200M reads per sample) to profile microbial and host transcriptomes. A total of 2208 bacterial species across 13 phyla and 165 genera were identified, with Proteobacteria and Actinobacteria dominating. Patients with DC and ACLF showed distinct microbial community structures compared with donors, with enrichment of specific gut, oral and respiratory derived taxa. Functional analysis indicated activation of Escherichia coli in ESLD, highlighting intrahepatic microbiome reprogramming and potential diagnostic and therapeutic value.
