Interleukin-17 inhibitors in the treatment of moderate-to-severe psoriasis - real world data

The study is a long-term, retrospective analysis of real-world clinical data collected between 2019 and 2024 at a dermatology referral center in Poland. Eligible participants were adult patients with moderate-to-severe plaque psoriasis, defined by a Psoriasis Area and Severity Index (PASI) ≥10, Body Surface Area (BSA) ≥10%, Dermatology Life Quality Index (DLQI) ≥10, or significant involvement of special anatomical sites (e.g., scalp, face, genitals, hands, feet, nails). All included patients had an inadequate response or contraindications to at least two conventional systemic therapies and had no prior exposure to IL-17 inhibitors.
Data were collected for 98 patients, distributed into three treatment groups: bimekizumab (n=48), ixekizumab (n=20), and secukinumab (n=30). Clinical outcomes were assessed using PASI, BSA, and DLQI scores, with safety and tolerability evaluated through documented adverse events. Statistical analyses were conducted using Statistica 13.1PL software (StatSoft, Tulsa, USA). No imputation was performed for missing data, ensuring a conservative interpretation of results. The data clearly support the hypothesis that dual IL-17A/F inhibition with bimekizumab offers superior clinical benefit compared to selective IL-17A inhibition. This is reflected not only in traditional clinical endpoints (PASI, BSA) but also in the mathematical modeling of long-term therapeutic response. Moreover, all agents were found to be safe and well-tolerated, reinforcing their value in clinical dermatology.
These findings provide actionable evidence for clinicians aiming to optimize biologic selection in the management of plaque psoriasis. Additionally, the modeling approach may serve as a template for future comparative effectiveness research. The dataset associated with this study offers a valuable resource for further investigations, including meta-analyses, real-world evidence syntheses, and pharmacoeconomic evaluations.
