①血液生化指标变化(红细胞、血红蛋白、红细胞沉降率、尿酸、甘油三酯升高、HDL-C下降),提示高原低氧环境暴露后机体为适应极端环境出现血红蛋白增多提高机体氧供,血液黏稠浓缩,相应出现红细胞沉降率下降表现,血脂、尿酸和白蛋白等代谢指标变化推测可能与平原地区居民对高原地区环境适应、热量消耗状态、能量供应方式改变等因素有关;②内皮功能下降:高原低氧环境还可能导致血管内皮功能的下降,而血管内皮功能的损伤正是各种心血管疾病的早期表现。③血管降硬度改善:高原短居后志愿者脉搏波传导速度明显减慢,该指标和动脉降硬度呈正相关,提示高原环境暴露后血管降硬度减低,可能与志愿者返回平原后血压下降相关。④右心室收缩功能损伤:超声心动图结合二维超声斑点追踪技术分析志愿者右心室心肌应变发现,早期反映心肌收缩功能的整体纵向应变指标下降,也提示高原环境暴露后右心室收缩功能受损。
A survey investigating the health impacts of high-altitude environmental exposure among short-term sojourn populations, conducted by Peking University Third Hospital. The Cardiopulmonary Function Group identified that following exposure to high-altitude environments, short-stay populations exhibited physiological adaptive alterations in blood biochemical indicators, vascular endothelial function, vascular stiffness, and right ventricular function.
1. Changes in blood biochemical indicators: Elevated red blood cell count, hemoglobin level, uric acid, and triglycerides, alongside decreased erythrocyte sedimentation rate and HDL-C levels. These changes indicate that to adapt to the extreme hypoxic high-altitude environment, the body increases hemoglobin production to improve oxygen supply, resulting in blood viscosity and concentration, accompanied by a corresponding reduction in erythrocyte sedimentation rate. Changes in metabolic indicators including blood lipids, uric acid, and albumin are speculated to be linked to factors such as the adaptation of plain-resident populations to high-altitude environments, alterations in calorie expenditure status, and shifts in energy supply patterns.
2. Impaired vascular endothelial function: The high-altitude hypoxic environment may also induce vascular endothelial dysfunction, which is an early manifestation of various cardiovascular diseases.
3. Reduced vascular stiffness: After short-term high-altitude sojourn, the pulse wave velocity (PWV) of the volunteers significantly decreased. As this indicator is positively correlated with arterial stiffness, the slowed PWV suggests that vascular stiffness is lowered following high-altitude environmental exposure, which may be associated with the reduced blood pressure of volunteers upon returning to the plain.
4. Impaired right ventricular systolic function: Echocardiography combined with two-dimensional speckle tracking imaging was applied to analyze the right ventricular myocardial strain of the volunteers. It was found that the global longitudinal strain, an early indicator reflecting myocardial systolic function, decreased, which also indicates impaired right ventricular systolic function after high-altitude environmental exposure.
