This series is comprised of a single volume, titled ‘Schedule H. Notifications of Cases of Infectious Disease (Public Health Act, 1902, Part III).’ A label in the front of the book reads: ‘Public Health Act, Schedule H. Regulation No. 17 - When any Local Authority is notified that a case of infectious disease is at a hospital within its District, it shall, after entering the notification in this Register, cause notice to be sent to the Local Authority for the District from which the case is reported to have been removed to the Hospital, in the form given in Schedule H.’ All but one of the cases recorded in this register were for Diphtheria, the other was for Scarlet Fever.The pages in this book are divided into two sections, one section is to be filled in an cut out and sent to the Local Authority, while the other half of the page remains in the volume. The section that is sent away has pre-printed information with blank spaces to be completed by the relevant authority. These pages read: ‘Sir, I have the honour to inform you that my Authority has this day been notified by Dr … a legally qualified Medical Practitioner, that … who was removed on … from … a place reported to be within the … is an inmate of … within this District, and is suffering from … I have the honour to be, Sir, Your Obedient Servant … Local Authority.’While the section that remains in the register is headed: ‘Public Health Act – Schedule H – Form 174.' And below lists the important information: Name of Patient, Inmate of (hospital), Removed from (address), Notified by Doctor (name), Suffering from (disease), Schedule H {Posted/delivered} to L.A. for (Shire) On (date).
