COVID-19 is a highly infectious disease with significant public health implications, including post-vaccination effects. Common symptoms include fever, fatigue, cough, breathing difficulties, and loss of smell or taste. Vaccines for COVID-19 were developed in less than a year, and understanding their post-vaccination effects is critical. This study presents PostVac, a dataset of 5800 individuals in Bangladesh, documenting self-reported post-vaccination symptoms. The dataset includes demographic information, vaccination history, binary symptom indicators, severity labels, and free-text responses. Symptom severity is classified as low, moderate, or high, based on the number of reported symptoms and their impact on daily activities. The dataset can support machine learning models to predict symptom severity and analyze post-vaccine impacts. All data were collected following ethical guidelines, including informed consent and anonymity. The dataset is available in CSV and Excel formats and can be used for statistical analyses and AI-based research on vaccine safety and public health outcomes.
