In this study, we uncovered that PAI-1, a key component of the prognostic biomarker panel, was largely secreted by fibroblasts and mesothelial cells. Molecular stratification of ascites using PAI-1 levels and STAT3 activation in ascites-treated cells highlighted a therapeutic opportunity based on a phenomenon of paracrine addiction. These results were recapitulated in patient-derived ascites-dependent xenografts (PDADX) which were generated by matched patient ascites and its cellular components. Whole exome sequencing of matched human peritoneal tumour, human ascites cells and PDADX tumours revealed consistent genetic features.
