Stroke Functional Dependency Nursing Assessment Dataset (SFD-NAD)

The Stroke Functional Dependency Nursing Assessment Dataset (SFD-NAD) is a large-scale multimodal healthcare dataset containing 585,716 stroke rehabilitation assessment records collected from distributed rehabilitation and nursing-care environments. The dataset includes heterogeneous patient information associated with post-stroke rehabilitation conditions, functional recovery status, nursing dependency observations, physiological summaries, mobility assessments, and clinical severity evaluations. The dataset contains demographic information, stroke-related clinical indicators, rehabilitation assessment measures, nursing-care observations, wearable healthcare summaries, cognitive evaluation indicators, and functional recovery measurements commonly used during rehabilitation monitoring and patient-support assessment. Clinical variables include Functional Independence Measure (FIM) scores, Barthel Index evaluations, National Institutes of Health Stroke Scale (NIHSS) scores, mobility indicators, transfer ability, feeding assistance requirements, communication ability, swallowing assessments, rehabilitation session information, fall-risk indicators, gait stability measures, cognitive assessment scores, and nursing dependency observations. The primary target label of the dataset is Functional_Dependency_Level, consisting of five rehabilitation dependency categories: Independent, Mild Dependency, Moderate Dependency, Severe Dependency, and Total Dependency.
