Iliocaval DVT Project Data Collection Raw Data

<i>Purpose: </i>This study evaluates anticoagulation alone versus endovascular intervention plus anticoagulation for symptom resolution and prevention of post-thrombotic syndrome and thrombus residual/recurrence in occlusive iliocaval deep vein thrombosis.<i>Material and Methods: </i>A retrospective analysis was conducted of patients with occlusive iliocaval deep vein thrombosis at a quaternary academic medical center between 2018-2023. Outcomes of interest included symptom resolution, time to symptom resolution, post-thrombotic syndrome, and thrombus residual/recurrence on follow-up imaging. Multivariable logistic regression compared outcomes between treatment groups, and an unpaired t-test compared mean times to symptom resolution.<i>Results: </i>A total of 76 patients were included, of whom 39/76 (51.3%) were female with a mean age of 51.4 years (SD 16.9). The anticoagulation alone group included 59/76 patients (77.6%) while the endovascular intervention group included 17/76 patients (22.4%). Patients treated with endovascular intervention were significantly more likely to have self-reported symptom resolution (odds ratio, 8.534; 95% confidence interval, 1.850 – 39.470; p = 0.006). No significant difference in the development of post-thrombotic syndrome or thrombus residual/recurrence was noted between interventions (odds ratio, 0.567; 95% confidence interval, 0.130 – 2.450]; p = 0.453) and (odds ratio, 0.825; 95% confidence interval, 0.210 – 3.280; p = 0.785), respectively.<i>Conclusion: </i>Endovascular intervention was associated with a higher rate of symptom resolution and a trend towards shorter time to symptom resolution compared to anticoagulation alone in patients with occlusive iliocaval DVT without differences in post-thrombotic syndrome development or thrombus residual/recurrence. These findings highlight the potential for broader clinical indications of endovascular interventions for patients with iliocaval DVTs.
