This work tackles an important blind spot in our field by exploring how shifts in gut microbes tie into both metabolic dysfunction and early Alzheimer changes in people living with type 2 diabetes. By combining detailed microbial profiling with standard clinical measures, we hope to uncover specific bacteria or pathways that flag neuroinflammation before dementia symptoms emerge. Ultimately, these findings could guide simple, gut-based strategies to help delay or even prevent cognitive decline in diabetic patients. CTRL A (Young Controls): metabolically healthy individuals aged 30-59 years, without diabetes or neurodegenerative disease. CTRL B (Older Controls): healthy volunteers aged 60 years or more, free of diabetes and other neurologic disorders. T2DM A (Young T2DM): patients with type 2 diabetes mellitus aged 30-59 years, with no history of neurodegeneration. T2DM B (Older T2DM): individuals aged 60 years or more diagnosed with type 2 diabetes mellitus. AD (Alzheimer Disease): participants aged 60 years or more with a confirmed clinical diagnosis of Alzheimer disease and without diabetes. AD-T2DM (Alzheimer + T2DM): patients aged 60 years or more with coexisting Alzheimer disease and type 2 diabetes mellitus.
