Social Determinants of Health (SDOH)

The AHRQ Social Determinants of Health (SDOH) database provides easy to use, linkable, SDOH-focused data to use in patient-centered outcome research, inform approaches to address emerging health issues, and contribute to improved health outcomes. The database contains more than 600 measures at the county scale (more than 300 at the census tract scale) drawn from 44 data sources in 5 domains (social context, economic context, education, physical infrastructure, and healthcare context). It includes annual data files and codebooks at three levels of geography (county, ZIP Code, and census tract), as well as a documentation file.
