This dataset contains anonymised survey responses collected from internship doctors as part of a study investigating the influence of enabling and enacting factors on patient safety, with physician self-leadership serving as a mediating mechanism. The dataset includes demographic variables (age, gender, internship duration, hospital type, and primary rotation), enabling factors (leader support, psychological safety, and safety knowledge), enacting factors (teamwork, error reporting intention, and information exchange), physician self-leadership behaviors, and patient safety outcomes. All constructs were measured using validated instruments.Data were collected between March 16 and 30, 2025, from internship doctors placed in Type C accredited hospitals in West Java, Indonesia, in accordance with the Ministry of Health Regulation No. 3/2020. This dataset supports the structural model analysis presented in the associated manuscript and may be used to replicate or extend the study’s findings. All personally identifiable information has been removed to ensure participant confidentiality.
