Three isolates were collected from a patient with carbapenem-resistant Klebsiella pneumoniae pneumonia, treated during a 32 days treatement with cefiderocol. Whole genome sequencing identified an unreported combination of mutations in the cirA and fiu genes, which encode siderophore receptors, resulting in phenotypic resistance to cefiderocol.
